Quad by MEDVi Review 2026: Formula, Pricing, Pharmacies and Refund Policy

Quad is a compounded product and is not FDA-approved. It has one published entry price. These figures were on quad.medvi.org on September 16, 2026.

The page describes QUAD™ as a “rapid-absorb” liquid that “dissolves under your tongue.” On September 16, 2026, it listed apomorphine, L-citrulline, sildenafil and tadalafil, without strengths, and claimed onset within 10 to 15 minutes and effects for up to 36 hours (company claims).

A MEDVi, LLC release dated July 18, 2026, labeled “a paid advertorial,” says: “Vardenafil and Apomorphine have been removed and replaced with PT-141 and L-Citrulline. Sildenafil and Tadalafil are unchanged.” The product page had not been updated to match, so we cannot tell which formula a new order would contain.

The release says “QUAD is a compounded prescription medication, not an FDA-approved finished product.” The Quad page carries no such statement; it tells users of compounded medications to talk to their provider and says “individuals may receive medication that looks different than what is portrayed on the website.” Compounded drugs are not FDA-approved: the FDA says it “does not verify the safety, effectiveness or quality of compounded drugs before they are marketed.”

Why the formula matters

Apomorphine and PT-141 are different compounds. In a November 2024 notice about VitalityXtra, a product sold on websites that FDA laboratory analysis found to contain sildenafil not listed on its label, the FDA also warns that this hidden ingredient “may interact with nitrates found in some prescription drugs, such as nitroglycerin, and may lower blood pressure to dangerous levels.” Get the current ingredients and strengths in writing and share them with your own clinician.

MEDVi, LLC says in its terms (updated March 31, 2026) that it “does not provide any Healthcare Services through the Platform and is not licensed to practice medicine.” The address it gives in its terms and on the Quad page, 131 Continental Dr, Ste 305, Newark, DE 19713, is listed in Stripe’s help center as the address of Legalinc, a registered agent. The medical consent names “CareGLP Affiliated P.C.s” and OpenLoop Health as the treating networks; no Quad clinicians are named. The BBB lists Elliot Gallagher as COO, and Moneywise, summarizing an April 2, 2026 New York Times profile, reports two employees and $401 million in 2025 sales.

The Quad page cites “multiple USA certified pharmacies” without naming the one that fills Quad. MEDVi’s home page lists Triad Rx (Daphne, AL), RedRock Pharmacy (St. George, UT) and Beaker Pharmacy & Compounding (McKinney, TX). The July 2026 release names Belmar Pharma Solutions, which calls itself “a national 503A compounding pharmacy and 503B outsourcing facility.”

On February 20, 2026, the FDA sent MEDVi Warning Letter 721455 about compounded semaglutide and tirzepatide on its website, saying the pictured labels suggested “MEDVi is the compounder of those drugs when in fact it is not” and citing misbranding provisions. The letter does not mention Quad or ED products. Compounded semaglutide and tirzepatide are not FDA-approved; the FDA does not review them for safety, effectiveness or quality before they are marketed.

Third-party reports we did not confirm in court or government records: Moneywise describes a California class action filed March 20, 2026 alleging MEDVi benefits from affiliate spam, and a 2025 Futurism report that site images appeared AI-generated; Implicator, summarizing a May 2025 Futurism report, also lists “fabricated doctor personas.” HIPAA Journal, citing the HHS breach portal, reports a January 2026 breach at OpenLoop Health affecting up to 716,000 people.

The Notice of Privacy Practices (effective January 1, 2024) lists a data protection officer contact in Bromma, Sweden, with a gmail.com address. A LegitScript link appears on MEDVi’s home page, but LegitScript’s public lookup requires a CAPTCHA, so we did not confirm it.

MEDVi’s terms say its services are “currently available to individuals located in certain states. To see the list of current states, please contact customer service at help@medvi.org.” No list appears on the Quad page, and the page does not list contraindications or eligibility criteria.

The published process is quick: a 3-minute intake, doctor review within 24 hours and free rush shipping in a plain package. The medical consent defines telehealth as care using an “audio-video interface such as videoconferencing,” but the Quad page does not say whether a video visit is required. Follow-up after the first order is not described.

Contact details differ by page. The Quad page lists hello@medvi.org and (585) 312-4226; the current terms and the home page list help@medvi.org and (323) 690-1564; the refund policy linked from the Quad page names support@medvi.org. The Quad page advertises “24/7 Medical Support.”

MEDVi publishes two versions of its refund policy. The current one, linked from its home page, says:

• “if you are disqualified by our healthcare provider for a medical reason you will receive a full refund.”
• Cancellation requests must arrive “at least 72 hours prior to your next billing date.”
• A full refund applies if you cancel “within the first 72 hours of your Subscription term and you have not received any Subscription Services.”
• For monthly plans: “YOU WILL NOT BE ISSUED A REFUND UPON CANCELLATION FOR THE MONTH IN WHICH THE CANCELLATION OCCURRED OR FOR ANY PRIOR MONTHS.”
• Multi-month plans are refunded “for the months for which no medication was shipped to you.”

The version linked from the Quad page (with terms dated January 1, 2024) has no 72-hour refund: “Other than cancellation due to medical disqualification, IN NO EVENT SHALL YOU BE ISSUED A REFUND UPON CANCELLATION OF THE SUBSCRIPTION SERVICES.”

Both terms versions require binding individual arbitration; the current terms allow an opt-out by email to help@medvi.org within 30 days of first accepting them. If you order, note your billing date and cancel in writing more than 72 hours before it.

The two BBB complaint figures come from different parts of the same profile, so we report both. Neither platform separates Quad from MEDVi’s weight-loss business, and the complaint summaries we read concern weight-loss orders: repeated or unexpected charges, difficulty cancelling, refused refunds and shipping delays. Positive Trustpilot reviews mention professional, responsive staff. Many “review” articles about Quad are advertorials published by or for MEDVi; we treat them as company statements.
